Services Provided Quality Mobile Labs LLC, provides mobile phlebotomy for the Hampton Road & nearby Areas in the state of Virginia, direct-to-consumer online lab testing coordination, and specimen collection for informational purpose and is not intended to be a substitute for professional medical advice. Quality Mobile labs does not perform diagnostic testing directly, give medical advice, diagnosis or treatment, and that test results are for personal informational purpose and should be reviewed with a qualified healthcare provider.

 

Note*****In some cases, if medical consultation or prescription-based services are needed, Quality Mobile Labs may assist clients by connecting them with our authorized provider. Any medical consultation, diagnosis, treatment, or prescription services are separate from laboratory tests purchases and will be solely between the licensed provider and the patient requiring further medical attention or care.

 

  1. Eligibility Users must be 18 years or older or have guardian consent and provide accurate information. 
  2.  Mobile Services: Appointments are required for Mobile Services. Clients must provide a safe environment. Services may be refused for safety concerns.  Payments are due in advance at the time of service non-refundable once completed. Missed appointment may occcur a fee. For cancellation schedule appointments please allow 2 hours for cancel mobile services to give the travel phlebotomist in route an advance notice.     
  3. Cancellation for Direct-To-Consumer Online Purchases: Please allow 24-48 hours for cancellations and direct deposit refunds to process. Processing times may vary depending on transaction status and payment processing through Square. 
  4. Test Results processing are provided by third -party labs such as Lap Corp & Quest Diagnostics and turnaround time may vary. Third-Party Services all lab testing is conducted by independent certified laboratories. Quality Mobile Labs LLC is not responsible for delays and errors in test results. 
  5. Patient Portal and Results Access through Crelio Health Services: Quality Mobile Labs provide patient access to laboratory test results primarily through the CrelioHealth, patient portal. Patients are responsible for creating and maintaining their own secure username and password to access their results through the portal. Patients may also access their laboratory results directly through the portal provided by the laboratory, including labcorp.com or questdiagnostics.com by registering for a patient account with the applicable laboratory. Quality Mobile Labs primary method of delivering test results is through the Crelio Health patient portal. 
  6.  Authorized Provider Role Authorized providers may only order tests and may not provide consultation unless arranged separately. The Authorized Provider is authorized to review eligible laboratory test orders. Providing medical oversight for laboratory direct-to- consumer online test purchasing.
  7. Authorization of Laboratory Test Orders: By purchasing laboratory testing services through this website, you acknowledge and agree that: Laboratory test requests are subject to review and approval by an Authorized Provider. The Authorized Provider may approve, deny, or request additional information regarding any laboratory order. Note****Orders are provided for informational and screening purposes only unless otherwise specifically stated. 
  8. No Physician-Patient Relationship: Use of this website and purchase of laboratory testing services does not create a physician-patient relationship between you and the Authorized Provider, except where expressly required by law or separately established through the providers direct medical care services. The Authorized Provider does not provide ongoing medical treatment, primary care services, emergency care or follow-up care through this website. 
  9. Test Result Disclaimer: Laboratory test results are provided for informational purposes and are not intended to replace professional medical evaluation, diagnosis, or treatment. Abnormal or unexpected results should always be reviewed with a qualified health provider or patient’s primary doctor. If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention. 
  10.  Limitation of Liability: Quality Mobile Labs is not liable for medical decisions, lab delays, or indirect
    damages.
  11. Cancellation Policy Cancellations: must be made in advance. Late cancellations or no-shows may
    be charged.
  12.  Website Users agree not to misuse the website or violate laws.
  13.  Changes to Terms Terms may be updated at any time with continued use implying acceptance.
  14. Compliance The Company and Authorized Provider strive to maintain compliance with all applicable legal, medical, privacy, and ethical standards, including state and federal laws governing laboratory testing services. 
  15. Privacy, Confidentiality, HIPAA, and PHI (PHI) Quality Mobile Labs LLC is committed to protecting the privacy and confidentiality of each patients’ Protected Health Information (“PHI’) in accordance and state privacy laws, including the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Any personal information, medical information, laboratory results, health records, insurance information, and other identifiable health data collected by Quality Mobile Labs may be used, stored, transmitted and disclosed only as permitted by HIPAA and other applicable laws and regulations for purposes including treatment, payment, healthcare operations, laboratory processing, and other legally authorized purposes. Quality mobile labs utilize secure systems and administrative, technical, and physical safeguards to help protect patient information from unauthorized access, use, alteration, or disclosure.  
  16. User Acknowledgement By using this website and purchasing laboratory testing services, you acknowledge that you; Have read and understood these terms; Understand the limitation of laboratory testing services and agree to seek independent medical advice when appropriate.
